Why did my reports fail to upload into Cornerstone?

In Cornerstone, patients can have any of the following statuses: 'Active', 'Deceased', 'Inactive', and the PDF-reports from SmartFlow still can be attached whether the patient in Cornerstone is 'Active' or not. But there are still several matters that could make an impact on the PDF report transferring back to PIMS/EMR.
Therefore the PDF reports might fail to upload to Cornerstone for these primary reasons:
1. Patient synchronization SmartFlow / Cornerstone was not successful.
2. The size of the PDF file is more than 50 MB.
3. Internet connection issues.
4. The DAT Staff ID was inactive in Cornerstone before sending the documents (more about setting DAT Staff ID active is HERE).
